Depending on the form, this may include a name, professional credentials, organization, contact information, website, service details, access information, and narrative responses.
Do not submit patient records, therapy notes, diagnoses, insurance identifiers, Social Security numbers, payment-card information, or other sensitive clinical or financial information through general website forms.
How submitted information is used
Submission information is used to review requests, communicate with submitters, moderate inappropriate content, maintain the directory and educational listings, and administer the organization’s programs. Provider submissions remain pending until reviewed. Public directory content may include information the submitter asked to publish.
Storage and service providers
Provider and education submissions may be stored in the website’s hosted database and transmitted to the organization through a form-notification service. Donation transactions are completed on PayPal; LGBTQIA+ Health NJ does not receive or store full payment-card details through this website. External services apply their own privacy practices.
Technical information
The hosting platform and security services may process routine technical information needed to deliver and protect the website, such as request timestamps, browser or device information, approximate network information, requested pages, and error or security logs. The website does not use this information to provide clinical services or create patient records.
Directory information and corrections
Provider-submitted information is treated as the submitter’s representation and is not rewritten merely for stylistic consistency. Providers may request administrative review or correction using the link on their profile. Visitors may report information that appears outdated or inaccurate. A request does not guarantee removal when retention is required for legal, security, or organizational-record purposes.

The website links to providers, government agencies, community organizations, crisis services, PayPal, and other external resources. Their privacy practices are separate from ours. The site is intended as a public informational resource and is not designed to collect private clinical information from children. A parent, guardian, or appropriate professional should assist when a minor needs to submit personal information.
